
A personalized wellness program to help you take control of your health
The SingulexWellnessTM Program supports patients and their physicians. Integral to the SingulexWellness Program is the involvement of the Clinical Health Educator (CHE) who is dedicated to serving both physicians and patients. The CHE formulates a Wellness Action Plan with the patient based upon the doctor’s treatment plan and the patient’s blood test results. Singulex provides the Personalized Wellness Program and CHE to both patients and physicians.
For patients:
Clinical Health Educators formulate a Wellness Action Plan for each patient based upon their blood test results and physician’s treatment plan. They provide personalized coaching in the areas of cardioprotective nutrition, medication compliance, exercise, weight, and stress management. Patients can also review their easy-to-read test results securely, at any time.
For physicians:
Physicians can securely obtain patient test results, view historical data, make treatment recommendations, and send messages to a patient’s designated SingulexWellness Clinical Health Educator. Patient compliance and accountability can be greatly improved through the education and frequent communication offered by Clinical Health Educators.
SingulexWellness 3-Stage Model:
- Provides personalized coaching in the areas of cardioprotective nutrition, medication compliance, exercise, weight and stress management.
- Enables each patient to implement their Personalized Wellness Plan through Remote Personal Touches with their CHE via telephone, email, or text messaging.
- Follows a unique SingulexWellness 3-Stage Model to educate each patient based upon understanding of their readiness to change.
- Improve patient compliance and accountability through frequent communication.

Singulex proprietary lab developed tests (SMCTM tests, sd-LDL, HDL2b, Adiponectin, and Leptin) were developed and performance characteristics were determined by Singulex.
