CVMedHome Personalized Wellness Program

Integral to the CVMedHome Personalized Wellness Program is the involvement of the Clinical Health Educator (CHE) who is dedicated to serving both physicians and patients. The CHE formulates a Wellness Action Plan with the patient based upon the doctor’s treatment plan and the patient’s blood test results. Singulex provides the CVMedHome Personalized Wellness Program and CHE to both patients and physicians.

The CVMedHome Personalized Wellness Program:

Provides unlimited personalized coaching in the areas of cardioprotective nutrition, medication compliance, exercise, weight and stress management.
Enables each patient to implement their Personalized Wellness Plan through Remote Personal Touches with their CHE via telephone or email.
Follows a unique CVMH 3-Stage Model to educate each patient based upon understanding of their readiness to change.
